How to get medical reports for supporting evidence

This page describes ways you can get proof of a medical condition for applications.

We require information about your medical history for some applications, for example, Blue Badge. You will need to upload the document when you complete an online application. That could be a photo you took with your smartphone or a scan you made with a scanner.

The type of evidence we require depends on the application, but how you can get it is the same.

With an NHS login, you can access your medical records via the NHS website or the smartphone app. You may be able to download copies of documents to use as evidence in your application.

Using the NHS app to access your medical information

With the free app, you can:

Contact your GP's reception team if you cannot access your medical summary online. There is no need for you to contact your GP.

Some surgeries may use a different app, but you can still use the NHS app for the same actions.
